12/3/2023 EnchantaVaughn Approved QMHPA greeted Chevelle upon arrival to the home setting. Upon arrival to the home setting, Chevelle appeared to be in a flat mood evident by her facial expressions such as a frown on the face. QMHPA inquired about her day. Chevelle verbalized to QMHPA that her day was going well. QMHPA informed Chevelle that this session was going to focus on stabilizing her mental health and recognizing and decreasing impulsive behaviors. Upon arrival, she did not present any symptoms of S/I. QMHPA began the session by inquiring about her current thoughts, feelings and emotions. QMHPA observed that Chevelle exhibited an agitated and sad character. QMHPA assisted Chevelle in identifying conflicts that can be addressed with problem-solving techniques. QMHPA utilized behavioral rehearsal methods that consisted of role reversal and education to educate Chevelle on interpersonal/personal problem solving skills which included defining the problems specifically and effectively, brainstorming the options, evaluating the options, choosing the options and implementing a plan, evaluating the results, and re-evaluating the plan. QMHPA moved onto address Chevelle’ pattern of impulsive and destructive behaviors. QMHPA directed Chevelle’ focus on the negative consequences of her behaviors to assist in reducing her thoughtless impulsivity. QMHPA utilized modeled behaviors to assist Chevelle in identifying and understanding the connection between her anxiety, depression, and impulsivity. QMHPA discussed and utilized modeled behaviors to assist Chevelle in becoming aware of the negative consequences her destructive behaviors caused for herself. Chevelle appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Chevelle verbalized her irritation with people not being there for her when she needed them. Chevelle used excessive profanity to describe her thoughts and feelings about the people she dealt with on a regular basis. Chevelle shared the various things she had done in her past and she was not proud of some of the things she had done to make money but she had done it to take care of herself.
